The continuing care plan includes the locations of 12-step meetings, referrals to supportive living and transition to outpatient treatment programs.
Our Intensive Outpatient Program is the perfect solution for clients requiring continuing care after their stay at a medical detox facility, inpatient treatment, residential treatment or partial hospitalization treatment program. The IOP program is offered 4 days per week on either a daytime or evening schedule (consisting of a three-hour group three times weekly and a one-hour life skills group). Intensive Outpatient Programs allow clients to arrange their group therapy and individual counseling sessions to work around their schedules.

All clients will be required to wear a mask and will have their temps taken before entering the vans or building. Clients will be no more than 10 per room. Clients who are at a distance will still be able to participate via telehealth.
Transformations Treatment Center’s Intensive Outpatient Program in Delray Beach, FL is an individualized treatment program for adults dealing with substance use disorders and mental health issues. IOP is offered for clients who wish to continue addiction treatment after a more intensive program has been completed, or for people who are seeking substance abuse treatment and do not require a higher level of care with the supervision of medical and clinical professionals.
Addiction treatment centers usually encourage clients in the IOP program to be involved in an outside support program such as AA, NA, or Smart Recovery, and depending on the program chosen, get a sponsor and do step-work at 12 step meetings while addressing triggers and cravings in drug or alcohol addiction treatment. We also offer an Outpatient (OP) program for anyone ready to step down from our IOP treatment program.
Our Outpatient Team offers IOP services and holds three hour group sessions, three days per week via telehealth. When combined with intensive outpatient care, treatment with medication is a viable option for treating alcohol and drug use as well as co occurring disorders. By providing treatment with medication, Transformations focuses on the long-term health and emotional well-being of our clients.
As part of the treatment process, new clients are assessed by the Transformations addiction specialists and medical team including Dr. Daud, our on-site, double board-certified Psychiatrist specializing in both General Psychiatry and Addiction Psychiatry. A thorough assessment determines whether they meet the criteria for treatment with medication. Clients participate in all clinical IOP services. Those clients taking medications on an intensive outpatient basis also attend regular medication management appointments.
While some mental health medications may be necessary ongoing for some individual therapy, in general, Medication-Assisted Treatment (to reduce cravings and withdrawal symptoms) is not intended to be a long-term solution. Rather, it is used to help an individual adjust slowly to life without drugs or alcohol. At the completion of the outpatient program, clients are referred by a member of our discharge planning team to one of our trusted medical partners for continuing care and medication management.
The ultimate goal for the use of Medication-Assisted Treatment is to help clients work towards a stable recovery that includes abstinence from drugs or alcohol.
Our IOP and OP programs in Delray Beach, Florida allow you to continue with normal daily routines while living either in local, supportive housing, or at home. Our evening IOP program allows clients to manage work, school or other responsibilities while participating in the program.
